Step 01
Initial nursing assessment
A registered nurse visits the home, reviews the discharge plan, and authors a personalized plan of care.
Step 02
Care plan & coordination
We coordinate directly with your physician and pharmacy. Family caregivers are looped in from day one.
Step 03
Ongoing skilled visits
Scheduled visits for wound care, IV therapy, monitoring, and education — with 24/7 RN backup.
Step 04
Progress reporting
Outcomes are tracked and reported back to the referring provider. Plans are updated as recovery progresses.
